Eligibility Requirements

- Request for assistance form from their website
- Budget sheet from their website
- Proof of current address (current lease or mortgage)
- Proof of all expenses (mortgage/rent payments, utility bills, child care, auto payments, etc)
- Documentation of any crisis (doctor’s note, fire report, receipt/bill of unexpected expense)
- Copy of photo ID
