Intake Process | Families that are interested in applying must call the main number and speak with the service coordinator for initial screening for eligibility. Only eligible applicants are scheduled to complete and application and an in-office interview. Adult applicants must bring a picture I.D., Social Security card, and Birth Certificate. For child applicants please bring children’s Social Security card, Birth Certificate, and shot records. |
